LASC Example

This sample worksheet illustrates how teams might move through the LASC blank worksheet. Different challenges and teams will have their own unique approach to this work, so use your judgment to identify which parts of the example are most helpful for your process. 

Background: In this fictitious example, the team completing the Learning Agenda process is interested in addressing the rates of uncontrolled Type 2 diabetes in their county, especially among communities of color who are experiencing disproportionately higher rates.

Events: what's happening?; Patterns of behavior: what are the trends?; Systems and structures: what influences the pattern?; Mental models: what values and beliefs shape the systems?
